CDC to fund 40 collaborations for addressing intersection between social determinants of health, chronic disease

The Centers for Disease Control and Prevention recently issued a notice of funding opportunities for collaborations between multisectoral partners to develop implementation-ready social determinants of health accelerator plans. CDC said it will fund approximately 40 applicants. The program is intended to reduce disparities in health outcomes related to chronic disease within a state, local, territorial, and tribal jurisdiction, community or catchment area.
